Requires health care plans and insurers to provide written explanations and hearing opportunities before terminating or non-renewing contracts with health care professionals, except in cases of imminent harm, fraud, or final disciplinary action.

Establishes that hearing panels must consist of three health care professionals licensed in New York in the same profession as the subject, with one appointed by the health care plan/insurer, one by the professional, and one chosen by the other two members.

Mandates that health care plans and insurers develop policies to regularly inform health care professionals of performance evaluation data and profiling information, with opportunity to discuss unique patient population factors.

Prohibits termination or non-renewal solely because a health care professional advocated for an enrollee, filed complaints, appealed decisions, provided reports, or requested hearings.

Requires notification within thirty days of termination/non-renewal request, hearing within thirty days of request, and minimum sixty-day effective date for terminations after hearing panel decision.
