Establishes distinctive "Alzheimer's Awareness" license plates available to New York state residents upon request, with design approved by the Department of Motor Vehicles commissioner.

Charges an additional annual service fee of $25 per plate, with revenues deposited to the Department of Motor Vehicles distinctive plate development fund up to $6,000 annually, and excess revenues directed to the Alzheimer's disease assistance fund.

Requires a $6,000 surety bond before plate production and issuance, which may be waived if at least 200 orders are received with advance payment of service charges.

Bond becomes discontinued if 200 or more plates are sold within two years; if fewer than 200 are sold, the Department of Motor Vehicles may recover a proportionate amount from the bond.

Takes effect 180 days after enactment, except provisions regarding bonding and regulatory implementation take effect immediately.
