Requires primary health care providers to give parents of children under six years old guidance on lead poisoning prevention, including contact information for state-designated childhood lead poisoning prevention programs.

Mandates primary health care providers conduct lead exposure risk assessment questionnaires for children ages six months to six years at routine well-child visits or at least annually.

Expands lead screening requirements to pre-school, pre-kindergarten, and kindergarten enrollees (in addition to existing child care requirements), with county health officers providing screening for families unable to obtain testing.

Authorizes schools to access blood lead information from the statewide immunization system to verify lead screening status for child care and school enrollment and to provide educational materials to parents about lead exposure risks.
