Amends the Property Condition Disclosure Statement form to require sellers of residential real property (1-4 family dwellings) to disclose indoor mold history to buyers prior to signing a binding contract of sale.

Adds indoor mold to the list of hazardous or toxic substances that sellers must disclose, alongside existing requirements for petroleum products, asbestos, lead, radon, and other environmental hazards.

Requires sellers to answer a new question (19-a) regarding whether the property has been tested for indoor mold and to attach test reports if available.

Takes effect 180 days after enactment and applies to all real property sale contracts entered into on or after the effective date.
