Creates a Community Residence Opportunities Waitlist (CROW) maintained by the Office for People with Developmental Disabilities for individuals seeking community residence placement who require that level of supervision and are eligible for the home and community-based services waiver.

Requires the office to distribute a CROW placement report at least monthly and upon request to service providers listing individuals actively seeking housing and determined to need certified residential opportunities.

Allows service providers with vacancies in supervised living facilities to review the CROW placement report and accept individuals for placement following a person-centered planning process, with placement making individuals eligible for Medicaid reimbursement and other residential funding.

Requires the office to act on CROW enrollment applications within 30 days, with individuals able to appeal denials to the commissioner within 30 days; the commissioner must conduct a hearing within 15 days and issue a decision within 30 days.

Automatically transfers all individuals currently on any community or certified residential opportunity waitlist maintained by the office to the CROW as of the effective date without requiring further action or office approval.
