Authorizes New York residents whose child, spouse, parent, sibling, or grandchild is actively serving in the U.S. armed forces to obtain distinctive "Blue Star Mother" or "Blue Star Family" license plates upon application and proof of active duty status.

Charges an additional annual service fee of $25 per distinctive plate beyond the regular registration fee.

Establishes the "blue star mothers and families support fund" in the state finance law to receive all revenue from the $25 annual service charges, with funds managed by the Division of Veterans' Affairs to support programs benefiting families of active duty service members.

Requires the Department of Motor Vehicles to obtain a $6,000 surety bond before designing and producing the plates, unless at least 200 advance orders with service charges are received within the required timeframe.

Takes effect 180 days after becoming law, with regulatory implementation authorized to begin immediately.
