Mandates reentry planning including Medicaid enrollment assistance, information on treatment facilities, housing, employment resources, and provision of one-week medication supply upon release from prison

Changes implementation timeline from 120 days to one year after the prior legislation (S.1795/A.533) becomes effective, with regulations authorized to take effect immediately
