Authorizes the Governor to present a "New York State Afghanistan War Commemorative Medal" to eligible service members in the name of the state legislature.

Eligibility requires being a New York resident (current or former) and serving on active duty in Afghanistan between October 7, 2001 and August 30, 2021 in the U.S. Armed Forces or organized militia.

Requires recipients to have been honorably discharged, released under honorable circumstances, or be a qualifying condition veteran or discharged LGBT veteran with a discharge better than bad conduct or dishonorable.

Limits each person to one medal and allows presentation to a designated representative if the eligible person is deceased before receiving the medal.

Takes effect 120 days after becoming law, with the Division of Military and Naval Affairs authorized to promulgate rules and regulations for medal distribution.
