Authorizes the Governor to present a "New York State Iraq War Commemorative Medal" to eligible individuals who served in the U.S. Armed Forces or organized militia on active duty in Iraq from March 20, 2003 through December 18, 2011.

Establishes eligibility requirements: recipients must be current or former New York State residents, receive an honorable discharge or honorable release, or qualify under specific provisions for veterans with qualifying conditions or discharged LGBT veterans (excluding bad conduct and dishonorable discharges).

Limits each recipient to one medal only and allows presentation of the medal to a designated representative if the eligible person dies before receiving it.

Directs the Adjutant General, in consultation with the Division of Veterans' Services, to establish rules and regulations for proper presentation and distribution of the medals.

Takes effect 120 days after becoming law, with the Division of Military and Naval Affairs authorized to promulgate necessary regulations prior to the effective date.
