Establishes a "reentry for the elderly program" for adults over 50 years old under community supervision or being released from state correctional facilities, using existing Department of Correction funds.

Requires the Department of Correction Commissioner to coordinate program implementation with the Office of Minority Health, NYC Department of Health and Mental Hygiene, NYC Department of Homeless Services, NYC Department of Social Services, and local stakeholders for participants in New York City.

Requires the Commissioner to coordinate with county health, mental health, and social services departments and local stakeholders for participants being released to counties outside New York City.

Program services include geriatric assessment and discharge planning, parole board interview preparation with residence identification, and case management connecting participants to housing, medical care, behavioral health services, benefits, employment, and senior center services.

Allows the Department to contract with non-profit entities to perform program functions, subject to available appropriations; takes effect 180 days after enactment.
