Requires sex offenders designated as level three (where victim was under age 18) to register address changes within 10 days and imposes penalties of up to one year imprisonment and/or $1,000 fine for failure to register.

Prohibits level three sex offenders from residing within school grounds (measured in straight lines from nearest entrance to property boundary) and prohibits all registered sex offenders from residing within a quarter mile of a victim's workplace or residence.

Mandates that probation departments, DOCCS, mental hygiene courts, and social services officials apply these residence restrictions when placing or supervising sex offenders and recommending their supervision conditions.

Requires courts to impose these residence restrictions as mandatory conditions of probation or conditional discharge for sex offenders convicted under specified penal law articles.

Takes effect November 1 following enactment.
