Requires all insurance policies providing hospital or major medical coverage to offer inpatient substance use disorder treatment coverage for a minimum of 45 days, including unlimited medically necessary treatment in residential settings.

Mandates outpatient substance use disorder treatment coverage for a minimum of 45 days under policies providing medical or comprehensive coverage.

Prohibits insurance policies from applying more restrictive financial requirements or treatment limitations to substance use disorder benefits than those applied to other medical and surgical benefits.

Extends the 45-day minimum coverage requirement to Medicaid-funded outpatient hospital and clinic services, as well as inpatient chemical dependence facilities.

Applies to all insurance policies and contracts issued, renewed, modified, altered, or amended on or after the effective date.
