Requires all voters casting ballots in person or by mail to present a current and valid government-issued photo identification, eliminating previous alternatives such as utility bills or bank statements.

Defines acceptable photo identification to include New York state driver's license or non-driver ID, U.S. passport, educational institution ID, military ID, government-issued medical card, or any identification issued by the U.S. or New York state.

Allows individuals without required documentation to obtain a free non-driver identification card from the Department of Motor Vehicles by submitting a signed affidavit and birth certificate no later than seven days before an election.

Requires county, city, town, and village clerks and state health departments to provide certified birth certificates at no cost to individuals presenting a signed affidavit for voting purposes.

Applies to voter registration applications, which must now be accompanied by a copy of valid government-issued photo identification.
