Establishes housing navigation services for individuals with developmental or intellectual disabilities, including developing individualized housing action plans, supporting applications for state and federal assistance, coordinating housing transitions, and creating housing sustainability plans.

Requires the commissioner to develop and implement a statewide housing navigation training curriculum, with authority to contract with qualified entities and conduct needs assessments in regions with greatest demand.

Creates a housing navigation task force co-chaired by commissioners from the Office for People with Developmental Disabilities, Office of Temporary and Disability Assistance, Office of Mental Health, and Office of Addiction Services and Supports, plus ten additional members with expertise in affordable housing and disability services.

Task force shall meet at least three times per year and provide recommendations on navigator training programs, research on housing navigation outcomes, reimbursement models, and expansion to other populations including high-utilization Medicaid service enrollees.

Authorizes submission of federal Medicaid waivers to fund housing navigation services and requires progress reports on service implementation effectiveness in the statewide comprehensive plan, effective April 1, 2025.
