Creates Article 32 of the Private Housing Finance Law establishing the New York Access to Home for Heroes Program to provide financial assistance for adapting housing for disabled veterans.

Defines eligible applicants as cities, towns, villages, or not-for-profit corporations with at least one year of existence and substantial experience retrofitting homes for persons with disabilities.

Authorizes the Housing Trust Fund Corporation to enter into contracts with eligible applicants providing grants up to $500,000 per contract, with programs to be completed within three years (extendable up to two additional one-year periods).

Limits any single municipality to receive no more than 50 percent of total annual program funds and allows eligible applicants to use up to 7.5 percent of contract amounts for administrative costs.

Requires applicants to submit plans addressing program feasibility, community impact, budget, completion schedule, and affirmative action and minority business participation goals.
