Grants the Department of Health Commissioner authority to set premiums adjusted by household size and income, establish deductibles and co-payments, and cap annual cost-sharing expenses while ensuring program viability and affordability.

Creates a 13-member Board of Trustees including the Health Commissioner and Superintendent of Financial Services as ex-officio members, with remaining seats appointed by the Governor (5), Senate President (2), Assembly Speaker (2), and minority leaders of both chambers (1 each).

Requires the Commissioner to submit an implementation plan within 12 months detailing enrollment timelines, coverage details, premium schedules, and benefits, and to seek necessary federal waivers for premium tax credits.

Establishes the New York State Public Health Care Option Program Fund to receive revenues from premiums, co-insurance fees, federal payments, and state transfers for program operations.
