Requires health insurance policies providing medical or major medical coverage to cover prosthetic devices, including repairs and replacements, for medically necessary rehabilitative and habilitative purposes as specified under 42 U.S.C. §18022.

Defines "prosthetic device" to include artificial limbs and all components and related services comprising prosthetic care, but excludes shoes and ordinary wearing apparel.

Allows prosthetic device coverage to be subject to annual deductibles and coinsurance amounts no greater than those applied to other essential health benefits.

Applies the requirements to policies issued, renewed, modified, altered, or amended on or after the ninetieth day following the bill's enactment.

Amends three sections of the New York Insurance Law (sections 3216, 3221, and 4303) to implement the mandatory coverage requirement.
