Permits municipalities (including cities, towns, villages, school districts, public authorities, and other governmental entities) to join county self-funded or self-insured health plans with consent from the county and the municipality's governing body.

Requires the county to file certification with the superintendent of financial services that the plan meets six requirements: mutual consent, 12% reserve fund minimum, 5% surplus account, specific stop-loss coverage with $200,000-$250,000 deductible, minimum 1,000 covered lives, and limited municipal liability.

Abolishes joint and several liability among participating municipalities; instead requires pro rata share assessments based on earned premium contributions to address plan impairments.

Grants the superintendent of financial services six months to review certification; if no objection is issued within that period, municipal admission takes effect automatically.

Requires counties to file annual certifications each January confirming the six requirements remain in effect.
