Adds definitions of "medical information" (information regarding an individual's medical history, mental or physical condition, or medical treatment or diagnosis) and "health insurance information" (policy numbers, subscriber identification numbers, unique identifiers, and application/claims history) to Penal Law Section 190.77.

Expands identity theft in the third degree (class A misdemeanor) to include obtaining medical information and/or health insurance information in another person's name.

Expands identity theft in the second degree (class E felony) to include obtaining medical information or health insurance information exceeding $500 in aggregate.

Expands identity theft in the first degree (class D felony) to include obtaining medical information or health insurance information exceeding $2,000 in aggregate.

Adds the same definitions of medical and health insurance information to the General Business Law Section 899-aa and State Technology Law Section 208, and takes effect 90 days after enactment.
