Establishes a co-navigation services program requiring social services districts to provide services to deaf-blind individuals to assist with physical access, environmental information, sighted guide services, and communication accessibility.

Defines "co-navigation services" to include visual and environmental information and communication in preferred language and communication modes, but excludes personal care, errands, decision-making, instruction, and formal interpreting.

Requires the commissioner to establish tiered reimbursement rates for co-navigators based on their training level and fluency in communication modes and sighted guide-mobility techniques for deaf-blind individuals.

Mandates social services districts ensure quality services by monitoring co-navigator compliance, developing additional funding sources beyond state appropriations, and providing training for both co-navigators and deaf-blind service users.

Authorizes the commissioner to establish an advisory committee including deaf-blind persons and stakeholders, and to promulgate necessary rules and regulations; takes effect 180 days after enactment.
