Allows qualified persons to register to vote or change voter registration address simultaneously with applications for hunting licenses, fishing licenses, or both.

Department of Environmental Conservation must design integrated forms (with State Board of Elections approval) that collect information for both licensing and voter registration without duplicating data.

Completed registration applications must be transmitted to the appropriate board of elections within 10 days of receipt, or by the 20th day before an election if received between 25-30 days before an election.

Forms must include voter eligibility statements, penalties for false applications, assistance availability, and confidentiality protections; applicants must affirmatively check a box to register or decline.

State Board of Elections must adopt rules, provide training to Department of Environmental Conservation employees, and develop public information materials; designates one agency coordinator to oversee the voter registration program.
