Candidates must submit a sworn, notarized statement within 15 days of filing their nomination paperwork (certificate of acceptance, designating petition, or independent nominating petition), attesting to military service, employment history for the previous five years, current residential address, residency requirements, and educational records.

Candidates must amend their statements if they make additional representations during their campaign, with amendments filed on the same date as their next campaign finance report filing.

Boards of elections must post all filed statements on their websites within five business days and publicly identify candidates who fail to file required statements.

Candidates who fail to file statements within five business days after the filing deadline face an initial civil penalty up to $1,000, plus $25 per day for statements more than ten business days overdue, with a maximum additional penalty of $1,000.

Penalties must be paid by the candidate personally and cannot be paid using campaign contributions or funds.
