Establishes a universal application form to be used by individuals applying for all public assistance benefits administered by the office of temporary and disability assistance.

Requires the universal form to list all information and questions necessary to determine eligibility for all public assistance benefits as appropriate or required by law, rule, or regulation.

Mandates local social services districts provide all public assistance applicants with the universal benefits form.

Requires the office of temporary and disability assistance to review the universal benefits form at least once every six months and add any additional information as needed.

Takes effect 180 days after becoming law, with regulations authorized to be adopted in advance of the effective date.
