Creates the New York Healthy Incentive Program (NYHIP) providing earned dollar amounts for SNAP beneficiaries to purchase fresh, locally-grown healthy foods from farm-to-consumer entities and similarly situated retailers.

Requires the Department of Social Services to apply for federal grants and waivers from the USDA to automate SNAP cards for community-supported agriculture subscriptions and to add accrued NYHIP incentives directly to SNAP cards.

Mandates the state hold SNAP beneficiaries and CSA subscription services harmless if beneficiaries lose eligibility during their subscription contract, with the state covering the full remaining contract value.

Directs the Department of Social Services to establish a mobile application, website, and interactive map identifying all participating farm-to-consumer entities and NYHIP locations searchable by town, city, county, or region.

Requires annual local outreach plans to inform low-income households about NYHIP eligibility and benefits through leaflets in public buildings, employment offices, food stores, media announcements, and translated materials in non-English languages.
