Expands the definition of "infertility" to include a person's inability to reproduce as a single individual or with their partner without medical intervention, and allows physicians to make infertility determinations based on medical, sexual, or reproductive history, age, physical findings, or diagnostic testing

Changes IVF coverage from "three cycles" to three completed oocyte retrievals with unlimited embryo transfers from fresh or frozen oocytes or embryos from covered retrievals

Requires IVF procedures to follow American Society for Reproductive Medicine guidelines, including use of single embryo transfer (SET) when medically appropriate

Prohibits insurers from denying coverage based on an individual's participation in fertility services provided by or to a third party (such as surrogacy arrangements)

Applies to large group health insurance policies and contracts that provide medical or major medical coverage in New York State
