Eliminates annual deductibles and coinsurance for primary and preventive obstetric and gynecological care, including cervical cancer screening, under health insurance policies and contracts.

Requires health insurance plans covering prescription drugs to include coverage for FDA-approved contraceptive drugs and devices as an added rider, with per-member premium rates standardized across contracts except for separate rating of small group policies.

Allows religious organizations and denominationally-affiliated insurers to opt out of contraceptive coverage if contrary to their religious tenets.

Permits individual enrollees to directly purchase contraceptive coverage riders at the same cost if their group contractholder declines coverage, with insurers required to provide written notice of this right.

Updates osteoporosis coverage standards to reference both detection and treatment criteria from Medicare and the National Institutes of Health; takes effect 60 days after enactment.
