Requires insurers to obtain and annually update names and residential addresses of all certificate holders under group health insurance policies.

Mandates insurers provide at least 30 days prior written notice of policy termination to each certificate holder by mail, unless the policyholder or group contract holder has already notified them or arranged substitute coverage.

Requires termination notices to include a complete statement of certificate holders' rights regarding coverage for illnesses and accidents before and after the termination date.

Prohibits group health insurance policy terminations from becoming effective until 30 days after the required notice is mailed.

Allows policyholders to request reimbursement from insurers for reasonable and necessary costs associated with providing termination notices to certificate holders.

Applies requirements to health insurers, accident and health insurers, and health maintenance organizations under New York insurance and public health law.

Takes effect 90 days after becoming law.
