Authorizes Janice Middlebrooks to file a retirement option election form with a joint allowance full option on behalf of her deceased husband William James Middlebrooks, who was employed at Albion Correctional Facility and was a member of the New York State and Local Employees' Retirement System

Requires the option election form to be submitted to the state comptroller on or before December 31, 2024

Deems William James Middlebrooks' disability retirement application effective November 1, 2004, retroactive to when he was advised he could no longer work due to cancer diagnosed in January 2004

All retirement benefits paid under this act shall go to Janice Middlebrooks and be in addition to any other death benefits previously paid

Estimated immediate past service cost of approximately $456,000 to be shared by the State of New York and all local participating employers in the retirement system
