Expands the definition of "home care" to explicitly include transition services from hospitals, nursing facilities, or other institutional settings to the home.

Adds social work, respiratory therapy, and nutritional counseling to the list of covered home care services provided by home health agencies.

Replaces specific visit limits (minimum 40 visits per year) with managed care provisions allowing preauthorization and utilization review, subject to appeal rights under Article 49 of the Insurance Law.

Broadens covered conditions to include medical management of any condition predisposing individuals to hospitalization or confinement in nursing facilities, or the need for other out-of-home services otherwise covered under the policy.

Takes effect January 1 following enactment and applies to all policies and contracts issued, renewed, modified, altered, or amended on or after that date.
