Authorizes the Commissioner of Mental Health, in conjunction with the Commissioner of Addiction Services and Supports, to impose a surcharge not exceeding 35 cents per access line per month on customers of all telephone corporations and voice over internet protocol service providers in New York State.

Requires service suppliers to receive a minimum of 45 days written notice before implementing the surcharge or any modifications to the surcharge amount.

Establishes the "9-8-8 suicide prevention and behavioral health crisis hotline trust fund" in the joint custody of the State Comptroller and the Commissioners of Mental Health and Addiction Services and Supports.

Directs all surcharge revenues to be deposited into the trust fund and expended only for costs related to administration, design, installation, construction, operation, or maintenance of the 9-8-8 suicide prevention and behavioral health crisis hotline system.

Takes effect 60 days after enactment, with immediate authority for regulatory agencies to adopt necessary rules for implementation.
