Authorizes New York residents to request distinctive "Brain Tumor Awareness" license plates upon payment of a regular registration fee plus a $25 annual service charge.

Creates the "Brain Tumor Research and Education Fund" in the joint custody of the Commissioner of Taxation and Finance and the Comptroller, funded by all $25 annual service charges from the distinctive plates.

Restricts fund expenditures to brain tumor research projects and brain tumor education projects approved by the Department of Health.

Requires the Department of Health to provide annual reports by February 1st detailing fund disbursements, award recipients, amounts awarded, and a financial plan for current and succeeding fiscal years.

Requires a $6,000 surety bond or 200 pre-orders to design and produce the plates; takes effect 180 days after enactment.
