Adds new section 511 to the Judiciary Law to exempt prospective jurors age 75 or older from jury duty upon their request.

Requires the chief administrator of the courts to prescribe a form for eligible seniors to claim the exemption and file it with the appropriate commissioner of jurors.

Makes exemptions permanent unless the exempted person requests reinstatement as a prospective juror using procedures established by the chief administrator.

Takes effect 180 days after becoming law, with authority granted to implement necessary rules and regulations immediately upon enactment.
