Amends Public Health Law Section 1356 and Agriculture and Markets Law Section 199-f to expand food allergy notices to include food intolerances in food service establishments and online food ordering services.

Requires the Department of Health to provide notices on its website in English and the 12 most common non-English languages, including procedures for handling customer food allergies or intolerances, cross-contact prevention, and allergic reaction response.

Mandates all food service establishments post the notice in a conspicuous location accessible to employees in languages spoken by staff, and include allergy or intolerance notices on menus (physical and online).

Requires food service establishments with online menus to provide customers with a method to indicate food allergies or intolerances or display allergy and intolerance information for each prepared food item.

Establishes a civil penalty not to exceed $125 per violation for non-compliance; the act takes effect 180 days after enactment.
