Creates a Department of Corrections and Community Supervision identification form to be issued to individuals upon release from correctional facilities, developed by the Corrections Commissioner in cooperation with the Motor Vehicles Commissioner.

Identification forms must contain the individual's full name, permanent address (if applicable), birth date, and any other information the commissioner deems relevant.

Allows the identification form to serve as sufficient proof of identity for applications for non-driver identification cards, eliminating the need for additional identity documentation.

Accepts the identification form as valid proof of identity for non-commercial driver's license or learner's permit applications that do not meet federal identification standards, without requiring additional proof of identity.

Takes effect 180 days after becoming law, with rule-making authority granted immediately for implementation purposes.
