Establishes a Medicaid program allowing family members of eligible enrollees to become certified complex care assistants within one year of federal approval, subject to passing an in-person examination administered by the Department of Health.

Requires private duty nursing agencies to pay all certification and training costs for family members, with no repayment obligations from the family member once certified.

Sets minimum reimbursement rates for complex care assistant services at $36 downstate and $33 upstate, or $42 downstate and $37 upstate for high-tech care services, with bi-annual cost-of-living adjustments based on the Consumer Price Index.

Limits program eligibility to family members caring for medically fragile children under 21 years old who qualify for Medicaid private duty nursing services and must work under the direction of a registered nurse.

Requires the state Medicaid director to submit a report to the governor and legislature within three years assessing the program's viability and making recommendations regarding its continuation.
