Requires the commissioner of the office of addiction services and supports to develop a statewide certification process for entities providing in-home substance use disorder treatment services by guidelines established by the commissioner.

Mandates applicants demonstrate ability to meet regulatory service standards under 14 NYCRR part 822, with out-of-state applicants required to register with the department of state and maintain a physical New York address.

Establishes a local governmental consultation process limited to one prior meeting where applicants must present a needs assessment and service proposal.

Requires certified entities to provide individual, family, and group counseling, plus admission assessments screening for gambling, overdose and mental health risks, safety plans, treatment recovery planning, medication for addiction treatment, naloxone provision, and toxicology testing.

Directs the commissioner to issue required guidelines within six months of the act's effective date.
