Allows pharmacists meeting specific education and experience requirements to adjust drug regimens, order disease-related laboratory tests, and perform patient monitoring functions under written protocols with physicians or nurse practitioners.

Requires written patient consent to participate in collaborative drug therapy management and mandates disclosure of the arrangement to all treating providers in the patient's medical record.

Takes effect 120 days after becoming law, with regulatory agencies authorized to implement necessary rules immediately upon enactment.
