Allows individuals to decline automatic registration by returning a postage-paid form within a specified timeframe; if they do not decline, they are registered to vote and receive notice with an opportunity to enroll in a political party.

Establishes parallel automatic voter registration procedures for Medicaid applicants and re-enrollees beginning June 30, 2027, through the Department of Health and county/city departments of social services, with a 15-day opt-out period if required by federal law.

Requires individuals registering within 30 days of an election to affirm they have resided in their county/city/village for at least 30 days, and provides monthly public reporting on registration numbers broken down by age and gender.
