Requires all trucks, tractors, and tractor-trailer combinations registered in New York with a gross vehicle weight rating of 26,000+ pounds and conventional cab configuration to be equipped with side under-ride guards when operated in cities with populations of one million or more on non-controlled-access highways, effective 24 months after enactment.

Defines "side under-ride guard" as a device designed to prevent pedestrians and bicyclists from falling into the space between front and rear axles and to prevent passenger vehicles from sliding under large vehicles.

Requires all state fleet vehicles with a manufacturer's gross vehicle weight rating exceeding 10,000 pounds to be equipped with side under-ride guards within 24 months of enactment.

Exempts state fleet vehicles, city fleet vehicles, and city-contracted vehicles from the private truck requirement, and authorizes the commissioner to grant exemptions for state fleet vehicles when installation is impractical or would disrupt public safety or health services.

Directs the commissioner to promulgate rules and regulations governing side under-ride guard specifications, installation, and inspection in consultation with the commissioner of transportation.
