Authorizes the superintendent of insurance to investigate fraudulent activities involving individuals operating motor vehicles without insurance coverage and insureds who misrepresent where their vehicles are garaged and operated.

Increases the civil penalty for knowingly possessing, producing, transferring, or using false insurance documents or forged insurance identification cards from a maximum of $1,000 to $2,000 for the first violation and up to $5,000 for subsequent violations.

Creates a new class E felony for "offering a false application for motor vehicle insurance or registration" when knowingly filing documents with false statements about residency or vehicle garage location.

Adds insurance fraud in the fourth degree as a class E felony for operating a motor vehicle on a public highway in New York with out-of-state insurance when the vehicle or owner are principally located in New York based on false residency statements.

Requires motor vehicle registration applicants to provide street addresses and sign attestations confirming the truth of their residence information; grants law enforcement access to vehicle and residence address information maintained by the Department of Motor Vehicles.
