Requires health care providers to inform patients of their right to make organ, tissue, or whole body donations and present the opportunity to register in the "donate life registry" during intake, discharge, or other care delivery processes.

Amends Public Health Law sections 2803, 2803-c, and 2984 to mandate that patients receive statements about anatomical gift registration as part of their episode of care at health care facilities.

Adds health care provider registration to section 4310 as an additional method for individuals to register for organ, eye, and tissue donations, alongside existing registration methods including driver's licenses, voter registration, and insurance enrollment.

Requires the commissioner to establish rules and regulations to implement these provisions and ensure space on required forms for patients to affirmatively register or decline registration in the donate life registry.

Takes effect January 1 following enactment, with immediate authorization for agencies to adopt necessary rules and regulations.
