Mandates practitioners wear photo identification name tags during patient encounters displaying their full name and professional title in large bold lettering, and verbally identify their profession during each clinical visit.

Prohibits non-physicians from using medical or medical specialty titles and requires practitioners with doctoral degrees who identify as "doctor" in clinical settings to clearly state they are not a medical doctor or physician.

Requires practitioners to provide patients verbally and in writing on first visit and annually with their full name, licensure status, profession, board certification, and links to state license verification websites.

Establishes a statewide public awareness campaign administered by the Department of Education (in consultation with Department of Health) to educate patients and the public about different health care practitioner types, their education requirements, and licensure verification resources.
