Establishes the "New York Long Term Care Trust Program" administered by the Department of Health to provide long term care services and supports benefits to eligible residents regardless of income or resources.

Requires employees and self-employed individuals to pay premium contributions beginning January 1st, two years after the effective date, with the initial rate set by the Comptroller at the lowest amount necessary to maintain actuarial solvency.

Eligible beneficiaries must be "qualified individuals" who have paid premiums for either 10 years lifetime, 3 years within the preceding 6 years, or 1 year if born before January 1, 1972, and must need assistance with at least two activities of daily living.

Provides a maximum lifetime benefit of 365 benefit units (initially $100 per unit, adjusted annually), covering services including adult day care, home care, nursing home services, assisted living, and various support services.

Establishes a Long Term Care Trust Commission with 27 members and an advisory panel to oversee program implementation, ensure solvency, adjust benefit units annually, and monitor program operations, with benefits beginning January 1st, five years after effective date.
