Expands IVF coverage in large group and individual health insurance policies to include three complete oocyte retrievals with unlimited embryo transfers from fresh or frozen oocytes or embryos, replacing the previous "cycle" limitation.

Mandates coverage for diagnostic tests and procedures necessary to determine infertility, including hysterosalpingogram, hysteroscopy, endometrial biopsy, laparoscopy, semen analysis, and blood tests.

Requires coverage for FDA-approved prescription drugs used in diagnosis and treatment of infertility, and ensures equal coverage whether fertility drugs are purchased through mail order or retail pharmacies.

Includes coverage for standard fertility preservation services when medical treatment may cause iatrogenic infertility, and prohibits discrimination based on age, sex, sexual orientation, marital status, gender identity, or disability.

Takes effect January 1, 2025, and applies to policies and contracts issued, renewed, modified, altered or amended on or after that date.
