Amends the Social Services Law to require all medical assistance payments to be capitated payments beginning five years after the act's effective date.

Authorizes the commissioner to develop patient rating categories based on risk to establish reimbursement methodologies and payment amounts.

Permits capitation payment systems to include contract arrangements such as incentive arrangements, pass-through payments, risk corridor mechanisms, and withhold arrangements as defined by federal regulation 42 CFR 438.6.

Requires the commissioner of health to submit draft legislation converting the medical assistance program from fee-for-service to capitated payments to the governor and legislative leaders within one year of enactment.

Directs the commissioner to immediately pursue federal waivers and submit Medicaid plan amendments necessary to implement the capitated payment system.
