Adds new section 4134-a to the Public Health Law allowing redaction of a physician's name from birth certificates when the listed physician's medical license has been surrendered or revoked by the office of professional medical conduct.

Permits requests for redacted birth certificates from either the person whose birth is documented (if 18 years or older), their legal representative, or the parent or legal representative of a minor.

Requires the department to issue a certified copy of the birth record with the physician's identity redacted upon receiving a request in the form or manner the department provides or approves.

Clarifies that no court order is required to authorize redaction of a physician's name under this section.

Takes effect immediately upon enactment.
