Establishes an Alzheimer's disease outreach and education program under the Office for the Aging in consultation with the Department of Health.

Program targets medical and non-medical professionals (physicians, nurses, social workers), consumers, caregivers, and organizations working with persons with Alzheimer's disease and related dementia.

Program purposes include promoting earlier dementia identification in health care facilities, serving as a clearinghouse for community support services (including caregiver support groups and respite care), and raising public awareness of Alzheimer's resources.

Program elements include educational materials in multiple formats, training curricula for institutions, public service announcements, and toll-free hotlines and electronic services for information.

Director shall maintain culturally and linguistically appropriate materials reflecting best practices, post information on agency and Department of Health websites, may establish advisory councils, and may accept grants and coordinate with public and private entities.
