Creates a uniform financial aid presentation that all New York schools educating students through twelfth grade must use when informing high school seniors about post-secondary financial aid programs, including FAFSA and aid for undocumented immigrants.

Requires schools to conduct oral presentations on financial aid to high school seniors at least once per school year, with all students receiving the same presentation regardless of aid eligibility.

Mandates high school seniors complete a uniform affidavit form (signed by student and parent/guardian, or by student if 18 or older) certifying attendance at the presentation and indicating intent to file FAFSA, apply for immigrant aid, or neither.

Requires affidavit forms to be available in the twelve most common non-English languages spoken by limited-English proficient individuals in New York, plus up to four additional languages at the commissioner's discretion.

Directs schools to collect completed affidavits and report data to the commissioner, who must compile an annual report on affidavit completion rates and FAFSA/immigrant aid application trends for submission to legislative leaders and posting on department websites by September 1 each year.
