Mandates distribution of closure notices to county officials, local legislative bodies, residents' councils, and community boards, with authority to convene meetings within 45 days to examine closure reasons and recommend approval, modification, or rejection.

Prohibits operators from closing until all residents transfer to alternative settings, increasing fees/charges, instituting new fees, or accepting new admissions after notifying the department of intent to close.

Requires operators to create plans for medical records management and continued resident care arrangements to be effectuated 60 days prior to closure, with full copies made available and updated on the department's website.

Imposes penalties under Public Health Law for operator non-compliance with any provision or commissioner-imposed conditions, effective April 1 following enactment.
