Creates the New York Healthy Incentive Program (NYHIP) to provide earned dollars for SNAP recipients to purchase fresh, locally-grown healthy foods from farm-to-consumer entities and similarly situated retailers.

Directs the Office of Temporary and Disability Assistance to develop a mobile application and website with an interactive map showing participating locations that accept SNAP benefits and NYHIP across the state.

Requires the office to seek federal waivers to automate SNAP card usage for community supported agriculture (CSA) subscriptions with weekly automatic deductions and to add NYHIP incentives directly to SNAP cards.

Mandates that SNAP beneficiaries and CSA partnerships are held harmless if benefits are lost during subscription contracts, with the state covering the remainder of the subscription contract.

Establishes a statewide outreach program requiring local social services departments to inform potentially eligible households about NYHIP through leaflets, media campaigns, and information provided to all new SNAP applicants in their home languages.
